​​​​POSITION TITLE: AAAD INTAKE COORDINATOR
PAY PLAN GRADE LEVEL: $49,618 - $54,716
REPORTS TO: AAAD CHOICES MANAGER
DUTIES AND RESPONSIBILITIES
1. Screen, assess, schedule and complete in-home visits and necessary paperwork for contracted TennCare programs which may include (but is not limited to) CHOICES, MDS Section Q, CLS, Medicaid, PERLSS, and NCI-AD. Ensure Intake Time frames are met.
2. Complete data entry, scanning, reporting and other administrative functions as needed.
3. Respond to referrals and requests for information; route individuals to appropriate programs.
4. Provide support for individuals and families in need, assessing risks and working with applicants and families to identify ways to prevent it from occurring; offer information on where and how to receive care, and assisting with eligibility and enrollment into the CHOICES program.
5. Serve as a resource for the long-term care community on issues related to CHOICES and other available programs.
6. Assist with staff training as assigned.
7. Maintain knowledge of community resources and provide education and information to applicants, communities, long-term care partners, and medical networks on a variety of topics related to long-term services and supports.
8. Participate in marketing activities that educate and promote long-term services and supports.
9. Establish networks and develop strategies to increase referrals and knowledge of the agency and long-term care options in the professional and lay-community.
10. Provide outreach and education about long-term care options for the community at large, consumers, caregivers, providers, the medical community, discharge planners, primary care physicians, and nursing facility social workers through community health fairs or events, printed materials, individual or staff meetings, and presentations or panel discussions.
11. Participate in training and implement new practices as directed by FTDD-AAAD/DDA/TennCare.
12. Represent the agency in a professional manner at all times.
13. Abide by FTDD Personnel Rules and Regulations as may be amended from time-to-time.

DESIRED KNOWLEDGE AND ABILITIES
Knowledge of Pre-Admission Evaluation (PAE) assessment and evaluation. Knowledge of social service programs and agencies. Ability to establish an effective working relationship with the public, medical community, contractors, grantor agencies, community agencies, service providers and colleagues. Experience in public speaking, planning, and conducting training. Possess command of the English language and ability to communicate effectively both orally and in writing.
EXPERIENCE AND EDUCATION
Licensed Practical Nurse, Social Worker, or Registered Nurse in the State of Tennessee. Minimum of two years experience in the medical field required. Experience working with elderly and disabled, compiling/submitting Pre-Admission Evaluation (PAE) applications, and knowledge of long-term services and supports is a plus. Proficient in usage of personal computers and appropriate software.
Position Title: CARING WORKPLACES WORKFORCE / EMPLOYER TRAINER
Pay Plan Grade Level: IV/ 1 ($52,099)
Reports To: Caring Workplaces Manager
Summary
The Trainer will play a key role in delivering training to employers participating in the Caring Workplaces Recovery to Work program. This position focuses on equipping businesses with the knowledge, tools, and strategies needed to become Recovery- and Reentry-Ready Workplaces, fostering inclusive and supportive workforce environments where individuals in recovery and/or reentering from the justice system are valued and empowered to succeed.
Duties and Responsibilities
1. Deliver all Caring Workplaces (CW) approved training courses to employers and their employees throughout eight counties of NETN and other counties as required.
2. Help create and design new interactive training courses on recovery and reentry friendly practices, workplace policies, and inclusive support systems to be delivered to employers and their employees.
3. Educate businesses on substance use disorder, recovery pathways, justice involvement, stigma reduction, and the benefits and value of second chance employment.
4. Facilitate virtual, in-person, and online training with employers, employees, and partners as needed.
5. Learn, manage, and make process improvements to the Caring Workplaces online training portal, Learning Management System.
6. Guide employers in developing and implementing supportive workforce policies for individuals in recovery and/or those with justice involvement.
7. Collaborate with CW staff, Peer Recovery Specialists, workforce development partners, community-based organizations, and CW partners.
8. Track engagement, collect feedback, and contribute to program evaluation and continuous improvement.
9. Provide input for promotional and educational materials for the Caring Workplaces.
10. Provide input and feedback for the Caring Workplaces Manager related to partner and sub-grantee work.
11. Provide input for appropriate grant reports and employer-required reports.
12. Promote the Caring Workplaces Rural Initiatives Opportunity through business and community engagement, outreach, and board participation.
13. Maintain up-to-date and consistent documentation in employer files, milestone trackers, and other required spreadsheets, including quarterly accomplishment data.
14. Communicate effectively with CW staff regarding employers to assist in the mission of navigating clients to gainful employment.
15. Abide by FTDD Personnel Rules and Regulations, as amended.
16. Always represent the organization in a professional and positive manner.
17. Perform other duties as assigned to support the mission of the program.
Desired Knowledge, Skills, and Abilities
• Knowledge and experience in professional development and/or training preferred.
• Knowledge and/or experience working with populations of individuals in recovery from substance use disorder and/or justice involvement, behavioral health, or workforce development.
• Strong written, oral, and listening communication skills.
• Excellent time management skills.
• Ability to work a flexible schedule as needed to fulfill job duties.
• Working knowledge of computers, Microsoft Office Suite, and other office and presentation software.
• Ability to facilitate sensitive topics with professionalism, compassion, and sound judgment.
• Passion for workforce development, behavioral health, reentry and/or recovery support, and community resilience.
Experience and Education
• Associate’s degree in marketing, human resources, criminal justice, social work, or a related field preferred; equivalent relevant work experience may be considered.
• Experience working with employers, HR professionals, reentry programs, or second-chance hiring initiatives preferred.
• Familiarity with Recovery Friendly Workplace initiatives, fair chance employment, or related workforce reentry programs preferred.
• Experience with training and facilitation for mixed audiences with sometimes sensitive topic materials preferred
