STRONG Families NETN TOPI Request Form
Filling out this form means you’re requesting enrollment in the STRONG Families NETN TOPI program.

To prequalify, you must meet the following: (1) be pregnant or the parent or legal guardian of a child under 18 that is living in your home AND (2) receiving or qualified for public assistance AND (3) live in one of 10 counties comprising Northeast Tennessee – Carter, Cocke, Greene, Hamblen, Hancock, Hawkins, Johnson, Sullivan, Unicoi, or Washington County.
Once completed, this form is sent to STRONG Families NETN staff who will contact you within 5 business days. The information you enter is completely confidential and there is no cost to you. You will be asked to provide general consent to be connected to a service provider in your area who’s able to meet a social care or medical need that you or someone in your care may have.
The Tennessee Opportunity Pilot Initiative (TOPI) is brought forth by the Tennessee Department of Human Services and TANF Opportunity Act. This initiative will help individuals, families, and the state of Tennessee by creating a new vision for the Tennessee social safety net and is dedicated to helping low-income families grow beyond their vulnerabilities. TOPI empowers all Tennesseans by partnering with local organizations to reach deep into their communities and work shoulder to shoulder with individuals and families, growing their capacity to take on life’s challenges and reduce their dependence on the social safety net.
